Intake Navigator - Access and Intake Program

Provide case management and service coordination to clients, regardless of their diagnosis, who are on a waiting list or may not yet be eligible for a needed service. Services are intended to assist the family or client to find or maintain stability while waiting for the desired service to become available. Intake Navigator supports may include the exploration of other services, community-based referrals and resources, brief treatment, and resourcing groups/peer support connections. Intake Navigators will be mobile throughout Chittenden County and coordinate with multiple stakeholders both in and outside of Howard Center to best meet client needs. 


  • Establish professional relationships with clients and families assigned to caseload:
  • Provides case management to children, youth, and adults with a variety of diagnoses
  • Provides follow up care and phone check-ins
  • Provides brief treatment (crisis stabilization, supportive counseling, skill building)
  • Transport clients in personal vehicle in the community when appropriate
  • Determination of appropriate services/referrals according to client needs
  • Assessment of ongoing need


  • 1 years' of experience working in Human Services, particularly in mental health, substance use, or developmental disability field
  • Knowledge of community resources, public benefits, diverse populations, crisis intervention/de-escalation, and/or lived experience 
  • Driver's license and use of a personal vehicle


  • This is a full time, benefit-eligible position

  • The salary for this position is $37401.00
