On-Call Sub - Safety Connection Monitor

This remote/home-based position is responsible for providing overnight coordination and implementation of residential independent living supports between the hour of 8 pm and 7am to individuals with a developmental disability, a mental health diagnosis, or desire to age in place. The Safety Connection Monitor will supervise, coordinate, and direct Safety Connection Bridge Monitors and Responders during shift hours. The Safety Connection Monitor is responsible for coordinating participant check-ins, initiating over-the-air outreach to clients, and in-person responses when indicated.  The Safety Connection Monitor will oversee the health, safety, psychiatric, and behavioral needs of Safety Connection participants and coordinate appropriate responses within the scope of the Safety Connection program.

Responsibilities:

  • Provide overnight independent living support to participants who check in nightly or weekly.
  • Manage and oversee over-the-air and in-person communication with participants.
  • Promote health and wellbeing of participants.
  • Monitor for safety and reinforce safe independent living strategies.
  • Manage home-based Remote Operator Station technology, maintain compliance with HIPAA rules, and protect participant PHI.

Requirements:

  • Five years of direct work experience in human services field or a combination of education and experience from which comparable knowledge and skill has been acquired.
  • A minimum of two years of experience working with individuals with developmental disabilities in residential services, mental health, or case management.
  • Supervisory experience.
  • Implementation of augmentative and facilitated communication supports
