Director of Client Care and Coordination

The Director of Client Care and Coordination is responsible for leading transitions of care across the agency and streamlining discharge planning and care coordination internally, with UVM MC and other community partners.  This position also leads coordination outreach and creative service delivery for under served populations and overseeing integrated access and intake.

Responsibilities:

  • Agency lead for transitions of care internally including utilization review process to ensure high quality care coordination and reducing/eliminating wait lists wherever possible
  • Create and maintain standards for discharge planning and care coordination
  • Liaison with UVM MC and other community partners related to transitions of care and decreasing ED utilization
  • Coordinate outreach, liaison relationships and creative service delivery for under served populations including New American/Refugee and LGBTQ communities in concert with Howard Center's Strategic Plan
  • Provide leadership to Partners for Access project, creating single point of access and public/private partnership for children's outpatient therapy in Chittenden County
  • Provide internal and external training on suicide prevention, intervention, assessment, care coordination and other topics as needed
  • Consultation including on-call supervisor rotation with crisis programs and/or other 24/7 programs
  • Member of Client Services Cabinet and Howard Center Leadership Team; close collaboration with agency leadership
  • Client Services liaison to Human Resources regarding internship structure, capacity and coordination
  • Assist in response to grant/funding opportunities with the Development office and other leaders
  • Lead clinical initiatives including Zero Suicide
  • Supervise Director of Access & Intake and ensure high quality, integrated and standardized access and intake systems across the agency

Qualifications:

  • Master's Degree
  • LICSW, LCMHC, Licensed Psychologist – Master's
  • 5 years' experience at a leadership level in a mental health, substance use, developmental disability and/or other relevant social service setting with proven client services, supervisory and program oversight experience
