Reach Up - Case Manager

Assist adults and families gain access to the mental health and substance abuse services needed to support employment and self-sufficiency. Provide integrated service planning, coordination and case management for men and women enrolled in *Reach-Up that suffer from mental health and/or substance use disorders.


  • Provide integrated case management and service coordination
  • Provide screening and ongoing assessment
  • Administer or refer to inter-agency resources for specialized screening and assessments for substance abuse, mental health, and trauma
  • Facilitate and monitor treatment plans as appropriate including necessary treatment team meetings
  • Integrate substance abuse and mental health services into Reach Up employment services
  • Assist to develop, place, monitor and evaluate participant's progress in substance abuse and mental health services as it relates to the Reach Up participant's employment goals
  • Coordinate with other services providers 
  • Support the attainment of work readiness skills and guide and support participants to work at their highest capacity

  • Salary starting at $37,401.00
