Intake Navigator:

Provide case management and service coordination to clients, regardless of diagnosis. Explore services throughout Chittenden County, coordinating with multiple stakeholders both in and outside of Howard Center to best meet client needs.

Responsibilities:

  • Establish professional relationships with clients and families assigned to caseload:
  • Provides case management to children, youth, and adults with a variety of diagnoses
  • Provides follow up care and phone check-ins
  • Provides brief treatment (crisis stabilization, supportive counseling, skill building)
  • Transport clients in personal vehicle in the community when appropriate
  • Determination of appropriate services/referrals according to client needs

Requirements:

  • One year of experience working in Human Services, particularly in mental health, substance use, or developmental disability field
  • Knowledge of community resources, public benefits, diverse populations, crisis intervention/de-escalation, and/or lived experience 
  • Driver's license and use of a personal vehicle

Structure:

  • Full time position

  • Annual salary of $37,401.00
