Community Case Manager

Provide case management services administratively and clinically to an assigned caseload of clients (adults 18 years or older) with major mental illnesses and co-occurring disorders. Supervision towards licensure available.

Responsibilities:

  • Determine quality interventions, maximizing resources and fostering the clients' independence
  • Deliver case management services in the least restrictive community environment respecting clients' rights to self-determination
  • Complete administrative and clinical reports

Requirements:

  • Bachelor's degree in a related field
  • 2 years' experience working with this population
  • Valid driver's License and use of a personal vehicle

Structure:

  • Full time position (37.5 hours/week)
  • Annual salary of $38,941.50
  • Exempt position 
  • Union position
