Case Manager - Chittenden Clinic Opioid Treatment Program:

Employees in this position may be eligible for up to $5,000 in grant-funded tuition or loan assistance

Work directly with clients struggling with opioid use disorder. Deliver comprehensive case management and health home services to clients in a clinical setting. Facilitate Chittenden Clinic's delivery of enhanced, coordinated services serving medical and psychosocial client needs.

The Chittenden Clinic is a large Opioid Treatment Program in Chittenden County, serving 1,000 patients. Our Medical Director, John Brooklyn, is a 2019 recipient of the Nyswander-Dole award for his contribution to the field of OUD treatment. We have a harm reduction orientation, use evidence-based interventions, and have a dedicated group of professionals using an inter-disciplinary team approach to client care. 

Responsibilities:

  • Provide case management services to patients as defined by the State Plan Amendment and in accordance with the Affordable Care Act. 
  • Work with treatment providers and community support groups with the goal of coordinating care and referrals
  • Customize frequency and nature of services based on client needs
  • Partner with treatment providers and community groups to coordinate client care and referrals
  • Facilitate educational groups

Requirements:

  • Associates degree or currently pursuing an Associate's degree from an accredited college or university
  • One year of relevant experience
  • Valid driver's license and acceptable driving record
  • Demonstrated clear professional boundaries
  • Discretion in assessing and delivering services; and in assessing need for triage
  • On the Roster of Nonlicensed & Noncertified Psychotherapists 
  • An Apprentice Addiction Professional Certificate must be obtained within 6 months of hire

Licensing Requirement Notice:

Vermont regulation mandates that if you do not hold a valid license required for this position then you must be on the Roster of Nonlicensed & Noncertified Psychotherapists before you can be hired for work in this position. If you are not yet on the Roster, please begin the process immediately to avoid delay in hiring. Note this process may take 3-5 days for in-state applications and up to several weeks for out-of-state applications. Instructions can be found on the website for the Vermont Office of Professional Regulation.

  • Annual salary starting at $42,604.38
