Care Manager - INCLUSION Program:

The INCLUSION Program serves children and youth in grades K-12 with serious emotional, behavioral, and mental health needs. A continuum of intensive supports and services are offered within the regular education environment, and also through tutorial services, to ensure that a student's needs are fully addressed without placing undue demands on the school's regular staff. Family services and summer programming are also provided.

The INCLUSION Program Care Manager would serve to support families in accessing needed medical, educational, social, or other services to address a child's needs and coordinate services across multiple community-based services.

Responsibilities:

  • Provides care coordination of services with multiple service providers including school teams to meet the therapeutic, clinical, and programmatic needs of each client
  • Maintains regular contact with families through phone calls, meeting attendance, conduct home visits
  • Assist families in identifying, procuring, and sustaining needed supports to ensure their ability to maintain functioning in school.
  • Conduct annual re-assessments for clients, maintain treatment related plans for clients and maintain agency required documentation.
  • Coordinate referrals for needed services and required applications or ongoing documentation for theses service

Requirements:

  • Bachelor's Degree in a related field
  • Must be on the Roster as a Non-Licensed, Non-Certified Psychotherapist (see licensing requirement below)
  • Basic understanding of Applied Behavior Analysis (ABA) Principles
  • 3 years of relevant experience

Licensing Requirement Notice:

Vermont regulation mandates that if you do not hold a valid license required for this position then you must be on the Roster of Non-licensed & Noncertified Psychotherapists before you can be hired for work in this position. If you are not yet on the Roster, please begin the process immediately to avoid delay in hiring. Note this process may take 3-5 days for in-state applications and up to several weeks for out-of-state applications. Instructions can be found on the website for the Vermont Office of Professional Regulation.

Structure:

  • Full Time (37.5 hours)
  • AFSCME Union (dues will apply)
  • Starting rate $23.22/ hour
