Care Manager - Early Childhood Family Mental Health

The Early Childhood Family Mental Health (ECFMH) program provides therapeutic support for young children and their parents, including parent supportive counseling and coaching; psycho-educational support around child development; and case management services. The care manager provides case management and administrative support to clinicians that serve parents, caregivers and their children, age birth to five, assigned to the ECFMH program. This position offers a combination of administrative responsibilities and direct service.

Responsibilities:

  • Provide support to clients by assisting clinicians in care coordination, thus maximizing their capacity to meet the client's treatment goals.
  • Provide support for processing referrals within the early childhood systems of care, including child cares and Children's Integrated Services.
  • Develop a knowledge base and capacity for navigating access to local resources.
  • Complete agency and program documentation and reporting.
  • Participate in staff meetings, group and individual supervision, case staffing, training and other program administration.
  • Work may be in an office, home or community-based setting.

Requirements:

  • One year of experience working in Human Services, particularly in mental health, Social Work, Psychology, substance use, or developmental disability field
  • Knowledge of community resources, public benefits, diverse populations, crisis intervention/de-escalation, and/or lived experience
  • Driver's license and use of a personal vehicle

Structure:

  • Full time
  • Hybrid in-person and remote
  • AFSCME Union (dues will apply)
  • Starting at $23.22/ hour
