Case Manager - Intensive Family Based Services

Provide intervention and case management to families through direct service, coordinating and collaborating with internal/external professionals and resources, and documenting services. Also assist in assessing family needs and developing Plans of Care, under the supervision of the Program Coordinator.

Intensive Family Based Services (IFBS) helps families when a child under 18 is at risk of being taken from the home because of severe emotional and behavioral issues, serious social problems at school or within their family, or for safety reasons. The program also provides support when children return to their home following an out-of-home placement.

Responsibilities:

  • Manage a caseload of approximately 4-8 families.
  • Direct service, including skill building, with youth in the home and in the community
  • Parent education
  • Case coordination, including communication with other providers, scheduling and attending team meetings, and documenting services
  • Establish relationships and partnerships with local resources
  • Crisis Prevention and management
  • Clinical documentation of services provided

Qualifications:

  • Bachelor's degree in a human service field
  • A minimum of 3 years' experience in working with children/youth, parents and/or families
  • A valid driver's license and use of a personal vehicle
  • The availability of working evenings and/or weekends
  • A demonstrated knowledge of working with substance use and addictive behavior

Structure:

  • Full-time position (37.5 hours/week)
  • This is an exempt position
  • This is a Union position
  • The salary for this position is $36,484.50
