Care Coordinator- Developmental Services

Care Coordinators are required to provide oversight of the defined support needs and to define and implement therapeutically appropriate crisis response plans as needed. They are responsible for oversight of individualized plans and for ensuring that all state and federal regulatory requirements are met so as not to jeopardize funding. 

Responsibilities:

  • Coordinate and manage services, programs or natural supports that meet the therapeutic, clinical and programmatic needs of each client
  • Participate in development and implementation of client's Individual Support Agreement
  • Use independent decision making and strong judgement skills during crisis situations
  • Communicate, organize and facilitate with a wide variety of people including: clients, families, support teams, community partners and potential/existing employers
  • Abide by Medicaid, state and federal laws
  • Good organization skills to maintain strict documentation requirements

Requirements:

  • Bachelor's degree in a field that allows for consideration as Q.D.D.P. (Qualified Developmental Disability Professional)
  • 2 years' experience in human services or combination of education and experience from which comparable knowledge and skill has been acquired
  • Budget management skills
  • Ability to utilize electronic health records system
  • Good writing skills for clinical documentation
  • Driving is required as well as an acceptable driving record

Structure:

  • This is a full time position of 37.5 hours per week
  • This is an exempt position
